Public Health Law requires these vaccinations in order for a child to attend school. Incomplete vaccination records will result in the student being excluded from school. A student who has received at least the first dose in each immunization series required for their age may be considered as In Process if they have age appropriate appointments within 14 days (or 28 days for live virus vaccines) of when the subsequent dose is due, confirmed with healthcare provider written documentation to complete the vaccine series according to the Advisory Committee on Immunization Practices (CIP) Catch-up Immunization Schedule and may remain in school. If scheduled appointments are not kept, the student is no longer eligible to remain in school.
Up-to-date immunization records must be submitted to the school within 14 calendar days of student’s entry (30 days if entering from out of state).
There are 3 exceptions. A child may be exempt from any or all of the required immunizations for the following reasons:

Seniors must have 2 doses of the meningococcal conjugate vaccine (MenACWY) or 1 dose if the dose was received at 16 years or older.
Check with your doctor to make sure your child is up-to-date or stop by and ask Mrs. Buscaglia in the Health Office. An up-to-date immunization form WILL BE REQUIRED or an appointment date scheduled to receive the dose will be due within 30 days of starting school.

A meningitis immunization is now REQUIRED to enter 7th GRADE. Check with your child's doctor to make sure they are fully immunized for the fall. An up-to-date immunization form IS REQUIRED to enter 7th grade and must be submitted within 14 calendar days of student’s entry.
This dose is required upon entering 6th GRADE. Check with your child's doctor to make sure they are fully immunized for the fall. An up-to-date immunization form WILL BE REQUIRED or an appointment date scheduled to receive the dose will be due within 30 days of starting school. This dose is required for students entering 6th grade and must be given at age 10 years or older to satisfy the Tdap requirement for students in grades 6 through 11. Doses of Tdap given at age 7 years or older will satisfy the requirement for students in grade 12.
NOTE: Students who are 10 years old in grade 6 and who have not yet received a Tdap vaccine are in compliance until they become 11 years olds. An up-to-date immunization form WILL BE REQUIRED or an appointment date scheduled to receive the dose within 14 calendar days of the minimum interval identified by the ACIP catch-up schedule by the 14th calendar day after start of school.
DTaP: 5 doses of diphtheria and tetanus toxoid-containing vaccine and acellular pertussis vaccine required for entrance or, 4 doses if the 4th dose was received at 4 years or older and series was started at less than 1 year old.
Polio: 4 doses or 3 doses if the 3rd dose was received at 4 years or older.
MMR/Measles, Mumps and Rubella: 2 doses
Hepatitis B: 3 doses
Varicella/Chicken Pox: 2 doses
Effective 6/13/2019, religious exemptions to immunizations will no longer be recognized in New York State for attendance at public, private or parochial schools (PreK through 12) or child daycare settings.
The Niagara County Department of Health offers immunization clinics on the 3rd Tuesday and 1st Thursday of every month (9-11:30 am)
