IMMUNIZATION UPDATE FROM NEW YORK DEPARTMENT OF HEALTH:
  
Changes in immunization requirements for school attendance take effect September 1, 2016.  
 
DTaP (Pre-K)
  •  4 doses
DTaP (grades K-5, except 6-12 as noted below*)
  • 5 doses of diphtheria and tetanus toxoid containing vaccine and acellular pertussis vaccine required for entrance.
  • If 4th dose received at 4 years of age or older, only 4 doses required.
  • Required for school entry. Range of 4 through 6 years of age no longer permitted. 
Polio (grades K, 1-3 & 6-9):
  • 4 doses of poliomyelitis vaccine (IPV)
  • If 3rd dose received at 4 years of age or older, only 3 doses required.
  • Required for school entry. Range of 4 through 6 years of age for 4th dose no longer permitted. 
Polio (Pre-K, grades 4-5, 10-12):
  • 3 doses 
MMR (Pre-K)
  •  1 dose
MMR (grades K - 12): 
  • 2 doses of measles and mumps vaccines and 1 dose of rubella vaccine
  • Required for school entry. Range of 4 through 6 years of age for second dose no longer permitted.

Varicella (Chicken Pox)
  • grades K-3 & 6-9 ... 2 doses
  • Pre-K, grades 4-5 & 10-12 ...1 dose
 
Hepatitis B (grades Pre-K - 12)
  •  3 doses, with correct spacing between doses
 
Hib (Pre-K)
  • 1-4 doses 
 
Pneumococcal/PCV (Pre-K) 
  •  1-4 doses 
 
*Tdap (grades 6-12)
  • 1 dose
 
Meningococcal (grade 7, 8 & 12)
  •  1 dose, grade 7,8
  •  2 doses or 1 dose if given at age 16 or older, grade 12
*Changes to Grades 6 through 12 School Entrance Requirements:
  • Children enrolling in grades 8 through 12 in 2015-2016 are in this cohort, including new entrants in subsequent years into the cohort's current grade levels (grades 9-12 for 2016-2017, grades 10-12 for 2017-2018, grades 11-12 for 2018-2019, and grade 12 for the 2019-2020 school year
  • Must meet immunization requirements of regulations in effect prior to July 1, 2014.
  • Do not need to have intervals assessed for immunizations.
  • Are compliant for 2015-2016 through graduation if they meet requirements in effect June 30, 2014, which were:
   DTaP/DTP      3 doses
   Polio               3 doses (grades 9-12)
   MMR              2 doses, or 2 doses of measles,
                          1 dose of mumps, and 1 dose of rubella
