  • NYS Immunization Requirements

    Public Health Law requires these vaccinations in order for a child to attend school. Incomplete vaccination records will result in the student being excluded from school.

    There are 3 exceptions. A child may be exempt from any or all of the required immunizations for the following reasons:

    • medical reasons, documented by a physician
    • serological evidence of immunity
    • have been diagnosed by a physician as having the disease mumps, measles, or chicken pox.  Parental statement of disease is not adequate proof and will not be accepted.

    Proofs of immunizations

    • The immunization record must be a medical document from a physician or the NYS registry, indicating the immunizations given and the dates of administration.  
    • Medical exemption requires a written statement from a physician licensed to practice in New York, stating that there is a valid medical contraindication to above vaccines.  Effective July 1, 2014, medical exemptions must be reissued annually.




     

    SENIORS Entering 12th Grade

    Seniors must have 2 doses of the meningococcal conjugate vaccine (MenACWY) or 1 dose if the dose was received at 16 years or older. 
    Check with your doctor to make sure your child is up-to-date or stop by and ask Mrs. Erck in the Health Office.  An up-to-date immunization form WILL BE REQUIRED or an appointment date scheduled to receive the dose will be due within 30 days of starting school.

    If you would like a copy of your child's immunization record, call Mrs. Erck at 716-778-6554 or have your child stop by the nurse's office and pick one up.


    Current 6th Graders, Going into 7th Grade

    A meningitis immunization is now REQUIRED to enter 7th GRADE.  Check with your child's doctor to make sure they are fully immunized for the fall. An up-to-date immunization form WILL BE REQUIRED or an appointment date scheduled to receive the dose will be due within 30 days of starting school.

     

    TdaP (Tetanus/Pertussis Booster) for 6th Graders

    This dose is required upon entering 6th GRADE. Check with your child's doctor to make sure they are fully immunized for the fall. An up-to-date immunization form WILL BE REQUIRED or an appointment date scheduled to receive the dose will be due within 30 days of starting school.


    PreK Requirements

    DTaP: -4 doses of diphtheria and tetanus toxoid containing vaccine and acellular pertussis vaccine required for entrance.
               -If 4th dose received at 4 years of age or older, only 4 doses required.
               -Required for school entry. Range of 4 through 6 years no longer required. 
    Polio: -3 doses of poliomyelitis vaccine (IPV)
               -If 3rd dose was received at 4 years of age or older, only 3 doses required.
               -Required for school entry. Range of 4 through 6 years of age for 4th dose no longer permitted.
    MMR, Measles, Mumps, and Rubella:
               -1 dose of measles and mumps vaccines and 1 dose of rubella vaccine.
               -Required for school entry. Range of 4 through 6 years of age for second dose no longer permitted.
    Hepatits B:
               -3 doses
    Varicella/Chicken Pox
               -1 dose
    Hib/Haemophilus Influenzae Type B Conjugate:
               -1 to 4 doses
    PCV/Pneumococcal Conjugate
               -1 to 4 doses


    Kindergarten Requirements

    Immunization requirements for school attendance changes took effect on July 1, 2020. 2 doses of measles and mumps vaccines are now required for Kindergarten. Children will no longer be able to wait until age 7 to meet this requirement.  


    Immunization Update from NYS Department of Health, NYS Education Department and Office of Child and Family Services

    Effective 6/13/2019, religious exemptions to immunizations will no longer be recognized in New York State for attendance at public, private or parochial schools (PreK through 12) or child daycare settings.

    Immunization Clinics

    The Niagara County Department of Health offers immunization clinics the 3rd Tuesday and 1st Thursday of every month (9-11:30am)                                at Trinity Lutheran Church 67 Saxton Street, Lockport NY 14094.

    Please call (716) 278-1903 to schedule an appointment. 

    *Please bring a copy of your child's immunization record - if you do not have one call your doctor or school nurse.

     

    If you have any other questions, please call your child's Nurse's Office.

    NYS laws require each parent/guardian to provide the school with an immunization record from your Health Care Provider, Health Dept. or an official immunization record from your child's previous school.  Dates must be included in the record.
